TERMS: All accounts are payable within 30 days of statement date. Accounts not paid within the stated terms will be subject to COD status and a late charge of 2 percent of the unpaid balance. Prices subject to change without notice.
LIMITED WARRANTY/LIMITATION OF LIABILITY. Neo Dental Laboratory, Inc ("the lab") warrants that all dental devices (a "device") are made according to your specification and approval in the belief that the device will be useful and MAKES NO OTHER WARRANTIES INCLUDING, BUT NOT LIMITED TO, ANY IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE.
Subject to the return of a device that is placed and then fails, the lab will repair or replace the device without charge for the cost of materials and workmanship or refund the original price paid, at lab’s discretion. (1) porcelain to metal, full metal, Anterior BruxZir, Bruxzir Esthetic, Lava Esthetic, Katana STML, Densply XT ML and custom titanium or zirconia abutments (excluding abutments with angulations greater than 20 degrees), up to 5 years; (2) BruxZir, Lava Plus, NexxZir PFZ ; up to 5 years; (3) full porcelain, single-unit inlay, onlay, veneer, and crown emax, upto 3 years;(4) thermoformed appliances and splints (if the failure is due to defects in materials or workmanship), provisional, composite resin Maryland and inlay/onlay bridges, up to six months.
Failed restorations are not covered by warranty if: (1) Imperfection was found on the impression and doctor instructed lab to proceed by estimating, approximating or if doctor marks/trims margins (2) Any bridges with a cantilever pontic(s) (3) Bridges with more than 3 pontics wth 2 abutments (4) Doctor instructed lab to alter prep on model/die to fix draw issues or path of insertion issues (5) Doctor instructed to proceed to finish restoration with indications that exceeds manufacturer’s protocol (6) Doctor instructed to reduce opposing teeth on model to fix insufficient clearance issue.
This Warranty is exclusively for your benefit, is not transferable and does not extend to any patients. You agree to pay all other costs of adjustment, repair and replacement of a device. Except where prohibited by law, the lab WILL NOT BE LIABLE FOR ANY LOSS OR DAMAGES ARISING FROM THE USE OF A DEVICE, WHETHER DIRECT, INDIRECT, SPECIAL, INCIDENTAL OR CONSEQUENTIAL, regardless of the theory asserted, including warranty, contract, negligence or strict liability and if such disclaimer is not permitted by law, the duration of any implied warranty is limited to 90 days from the date of delivery. In the event of a dispute and absent an amicable resolution the parties mutually agree to waive class actions in favor of mandatory individual arbitration of claims under this limited warranty in and in accordance with the laws of California. The lab does not guarantee the performance of independent carriers.

What is covered?
Restorations will be repaired or remade at no charge if the workmanship or material is faulty. The warranty starts from the invoiced date.
What is not covered?
- Cash refunds (we will gladly repair/remake within the time frame)
- Repairs resulting from accident, neglect, abuse, wear and tear
- Change of treat plans (shade, material, indication changes)
- Improper adjustments or dental hygiene
1. The original dental restoration must be returned
- If the product is damaged, please send any portion of the device available.
- If the product is lost, please call Neo Dental Laboratory
- New restorations will be invoiced, until the failed product is returned.
- If the product is being used as a temporary, simply return it and it will be credited.
